You need to download and print a copy of your filed personal tax extension form. After you print, follow the steps below:

Enclose the copy of your printed form 4868 with the check/money order.
Write your Social Security Number (SSN), daytime phone number, and ‘(tax year) Form 4868’ on your check or money order.
Do NOT staple or attach your payment to Form 4868, the IRS will not accept it.
Mail your check or money order with a copy of your Form 4868 to the "United States Treasury" based on your location.
