You need to download and print a copy of your filed personal tax extension form. After you print, follow the steps below:

• Enclose the copy of your printed form 4868 with the check/money order.
• Write your Social Security Number (SSN), daytime phone number, and ‘(tax year) Form 4868’ on your check or money order.
• Do NOT staple or attach your payment to Form 4868, the IRS will not accept it.
• Mail your check or money order with a copy of your Form 4868 to the "United States Treasury" based on your location.
