The State of Maine doesn't require businesses (LLC as Corporation or S-Corporation and partnerships) to file a separate extension form, it automatically grants extensions of up to 6 months to file your return as long you paid 90% of your Maine tax liability by the actual due date of the Maine business income tax return.
Extension Form to use
There is no state-specific form to request an extension. Businesses can file a federal Form 7004 to obtain an extension with the state of Maine. Maine allows an automatic extension equal to any federal extension or 6 months, whichever expires later.

Any Special Requirements?
A tax extension does not give you more time to pay Maine tax due. As long as 90% of your Maine tax liability is paid by the original deadline you will automatically receive more time to file your business income tax return.
